Anna May Smith, 71 of Beaver Dam, Ky. passed away Saturday, May 7, 2016, at the Signature Health Care of Hartford in Hartford, Ky. Anna was born March 16, 1945, in Evansville Indiana to the late Earl Thomas Phelps Sr. and Juanita Wilson Phelps and wife of the late Nealie H. Smith Sr. She was the greatest mother in the word and a homemaker. Other than her parents and husband she is preceded in death by one son John T. Smith, two brothers Roy and Sam Phelps and one sister Linda Vanclaven.
Anna May Smith is survived by one daughter Angela Phelps and husband Chris of Beaver Dam, Ky., one son Nealie H. Smith Jr. and wife Jayme of Beaver Dam, Ky.; two sisters Sandy Guerrero and husband Anthony of Hartford, Ky. and Dixie Zentmire and husband Ralph of Evansville, Indiana; one brother Earl Thomas Phelps Jr. of Evansville, Indiana; 5 grandchildren; 7 great-grandchildren and several nieces and nephews.
Funeral services will be held at the Jones Funeral Chapel Thursday, May 12, 2016, at 12:00 pm with Bro. David Beliles officiating.

Burial will be in the Bethel Cemetery.

Visitation will be held on Wednesday, May 11, 2016, from 4:30 pm - 8:00 pm and Thursday 8:00 am until funeral time at 12:00 pm at the Jones Funeral Chapel.
